Wow, thats pretty bad... I have a couple more questions.
Was this a new barrel?
If it was not, how is its condition? I mean is it clean? How the muzzle look? You need to look close there could be a ding in the crown. You also may want to do a chamber/throat cast and see what it looks like in there.
If thats OK I would remove the scope and mount. Clean everything and re-install with lock tite and check that scope. Using a colimator move the POI up then over than down then back up. making the four corners of a box. Use about ten clicks each and you should see the POI make a nice box and with the last adjustment see the crosshairs return to origional position.
If no colimator is avalible, swap the scope for a known good one and go shoot it again. You dont need to site in, just be able to make a group.
Reading you explination, I suspect the barrel or scope to be your problem.
