Sounds like everyone starts with 'pipe' and then wakes up and figures out what is and is not safe.

I was told once that in relation to catastrophic accidents, all of the experts are dead.The inference is that on their way to the Pearly Gates they realized the folly of their last accident. Hope nobody gets hurt.
