How do you go about making a pattern?  Figure you use paper rather than leather as it would be cheaper to get it all correct

And do you add some if the holster will be lined?

I follow the directions in Al Stohlman's book How To Make Holsters when making a pattern. I don't add any extra when lining a holster but I bend the boot of the piece and glue the lining along the bend to avoid wrinkles, then I cut the lining to the shape of the piece and stitch it in by hand.

I too use the All Stohlman method however as opposed to some when I make a lined holster I make the lining as the holster and cover it as opposed to making a holster then lining it this also avoids wrinkles it is basically making a roughout holster then covering it with another piece
this makes sure the boot of the holster is not going to be too small as well, when lined and complete, I leave extra material on both then I sew it up then trim it out.
