 Obamacare Reality: Doctor Shortage on the Way  by Kate Rogers
Published April 17, 2013
FOXBusiness          With 30 million new people expected to enter the health-care system in 2014 under the Patient Protection and Affordable Care Act, experts say a looming doctor shortage isn’t a chance—it’s a fact.
“These 30 million new patients have either not gone to the doctor or [have been] going to the emergency room so that is putting pressure into the system. You have a foundational supply and demand shift,” says Mitch Rothschild, CEO of Vitals, a consumer tool physician evaluation company.
He expects the shortage to hit the primary care physician (PCP) arena the hardest and explains that there is approximately one PCP for every 15,000 people in the U.S. but come 2020, there will be about 70,000 less doctors available to consumers as a direct result of the law.
In fact, a recent Deloitte 2013 survey  of U.S. physicians found 57% doctors view changes in the industry under health-care reform as a threat, and six in 10 physicians report it’s likely that many will retire earlier than planned in the next two to three years, fueling the shortage.

Here’s the Doctor, You Have 6 Minutes
Here’s the problem: the number of people in the patient pool will increase 12% next year, but the supply of doctors will hold steady. Patients are already waiting longer to see their physicians and spending less time with them than they have in the past, according to Rothschild. On average, Vitals found patients are waiting 6% longer to see their doctors and spending only six minutes in the exam room.
“You’re going to wait longer and spend less time with doctors,” he says.
Brookings Institute Senior Fellow Henry Aaron says to expect a “catch-up period” when the new patients enter the system and start seeing doctors for issues they had previously left untreated.
“Expect this to occur over several years because when people are newly insured, they finally have access to care,” Aaron says.
The cost of seeing a primary care doctor shouldn’t be impacted for the most part, Rothschild says. Fees are set by insurance companies, so while doctors will be stretched thinner, primary care physicians likely won’t up their prices.
“There are currently about 9,000 urgent care centers across the country that didn’t exist 15 years ago,” he says. “Most are run by ER doctors.”
Specialists are another story: Rothschild  warns their prices will go up and that medical students are entering this field to cash in on the trend.
“These specialists have gone up in price, and that is where doctors are heading because that is where the money is,” he says. “It’s like Wall Street.”
The Aging Population
Older patients are going to feel the brunt of the doctor shortage because according to Aaron, there is a current lack of doctors that specialize in geriatric medicine. The ACA will be causing a shortage of PCPs at the same time the aging population will be seeking more help.
“The Affordable Care Act will have a marginal impact on primary care, but will just be uncovering a [existing] shortage by making services equally available to people who were previously uninsured,” Aaron says. “I think the most serious shortage has nothing to do with the ACA, it’s more to do with the population aging- the elderly being newly covered is negligible.”
Young patients will still have experienced doctors to visit—although it might be hard to get an appointment and not long to speak to the doctor. “As the number of elderly increases, they will be forced to go to people that aren’t as well trained in their illnesses,” Aaron says.
More elderly are turning to minute clinics offered at drugstores like CVS because of their convenience and availability. “No one is getting the flu shot from their doctor—they are going to Walgreens and CVS,” Rothschild says. “Those are serving the more elderly.”
A Nursing Boom
Just a few years ago, the medical system was facing a nursing shortage, incentivizing many people to become registered nurses. However, now they are graduating into an evolving health-care system with fewer physicians.
Rothschild says the move may benefit from the shortage, and step up to run these clinics and medi-merge facilities.  Oftentimes, and especially as more pressure is put on the system, these nurses may be the only people seeing patients as doctors have less and less time.
“They will need nurse practitioners,” he says. “This is just another way the system is adapting.”

Powderman, read this in several different articles. Including I believe the WSJ. Also talked with several physicians who are in complete agreement. More patients,less money, and so many are pushing the limit now. Many are considering retiring early, just to avoid the mess. Yep, ObieWanKnobie opened up a can of worms with this one. gypsyman

My son-in-law is a doctor who also teaches medicine at a well known university and also teaches ER medicine at the local hospital. His classrooms are packed with a waiting list to get into his classes. If there's going to be a doctor shortage, somebody is going to have to start killing doctors as there's tons of them coming thru the college lines. Most are having issues with finding hospitals to do their residency as they are packed as well. While I think Obamacare is the worst thing to happen to this nation since Pearl Harbor, this is nothing but propaganda to rile the masses. Obamacare doesn't need to "threaten" your doctor relationship or create an atmosphere of assembly line medicine to know it was bad legislation and needs to be repealed.

Part of my job is to to set up observations for students in our health science classes. The four five area hospitals I have worked with are loaded with med students and we have to work around that. Just my observation.

I disagree with the shortage of doctors in the U S. Show them your availability to pay and your in. Its the illegals and ones who abuse the system for non major treatments that could be done at a home that chokes up valuable hospital time.

This was expected , my doctor told me that to practice medicine someone who was educated in another country had to pass a test to practice in the USA. Obama had the passing score lowered to allow more doctors to pass and fill the open slots. It has been expected doctors fed up with the new HCA would leave the profession .
 As for show them the money - my doctor is starting a deal where you pay a sum of money up front to make sure you get appointments when you want them and longer office visit times.

Likewise,  there is going to be a shortage of teachers because of Scott Walker's actions in
Wisconsin. The cuts in teachers's salaries means fewer people will be studying to be educators.

They aren't paid for their summers. They are paid based on days of contacted work unlike construction workers who draw unemployment while getting their winters off.  I guess Dr. wages  will also come down to wages more in tune with others.  Personally, I prefer to have my heart surgeon and my son's teacher to be well paid.

There pay in many cases is paid year round. Call it as you wish. As for construction workers what's up with that ? You say construction worker do you know how broad a segment of the work force that covers ? They get paid from minimum wage to over 6 figures . So which are you comparing ? Some have both more education than a teacher and more practical experience. I will use myself as example. I have college, plumbing school, HVAC school, elec. school, welding school both elec, and gas, Lic. in two states and enough classes on different piping systems and HVAC systems to last a life time. On top of that I have to be tested every 2 years on the trades I hold a lic. In . Did I mention I taught school for 8 years in public schools. When teachers start getting paid according to the quality of the students they promote then we can compare them to the construction worker.
 Well paid is not the same as over paid. I really don't see a teacher making the same investment as a heart dr. so I can't see them on the same level of pay. When I see what teachers turn out as grads I must consider their pay in many cases out of line with their ability. There are good teachers for sure but there are also bad ones .

That's not quite true here. Yes they are paid for contracting for a specific amount of days but here they average the pay out over 12 months. So the teachers get a paycheck even during the summer. My significant other works for the 5th largest school district in Texas. What I find interesting is the teachers pay in various parts of the country. Here, a good wage of 40 grand a year is like hitting the lottery ..almost. Seasoned teachers here will make that. I've heard of teachers in the north making twice that and complaining about it. The Dept Heads here don't make that kinda money. But we do build some fantastic school buildings with millions in architectural detail.


I don't see where cutting teachers pay in Wisconsin is going to effect anything. Teachers, good teachers  don't do it for the big paychecks. They do it because they love it. Our school district hires from the entire nation, not just Texas. Wisconsin isn't any different.


In regards to lowering test scores for out of nation doctors, that might be true but it isn't going to work here. Schools are so over crowed with up and coming doctors that those that can't keep up are dropped to make room for those that can keep up. There's no mercy in how they drop a student- you don't make the grades,yer out- conversation is over. So those that are coming out as doctors are the best we can turn out. The out of nation poor scoring docs will be left to doing vitals, reading tests, etc and maybe even open their own doc in a box. But they're not going to take over our medical system.

Public employees get paid what the public in that area can afford....period. If you pay them more than your taking in, you build debt. Building debt is not good. In our area teachers get paid well, they had to make some adjustments, because taxes were not coming in, because people are out of work, so they made adjustments to staff size as well. Example...California teachers may make a lot more than teachers in Alabama, of course this is very normal, you can't raise teachers pay in places where the money is not there, so obviously you can't make all teachers in the country the same.  All jobs in California pay a lot more than in Michigan, because California is so messed up. Teachers are public employees, they should be paid what can be afforded without going into deep debt. The unions...well...another post for that, but unions are not intersted in what can be afforded, they have their own little business going on the paychecks/backs of those they "serve"...if they were worth their salt people would not leave them so fast when they get the chance. That kind of speaks for itself.
Doctors...already had a shortage, add 30 million people, yup...a bigger problem. Real big problem. Even before this HC kicks in, you can't hardly get in to see a doctor in my area.
People become Doctors because they want to...it's hard work, people become teachers because they want to, and people cut trees down for a living because they want to..we do what we want to do, if it's all for the money only you won't be happy with much of anything. I told my kids to do what they dream about doing and forget the money, they will be happy. It worked out for all of them. I guess if your a teacher and don't like what your area pays, do something else...stop crying over it. If you can't get by on the teachers wages in this state, your budgeting like a socialist and won't ever have enough money anyway.

Really it saves taxpayers money.  Just like there are good/bad construction workers and doctors, teacher run the same gamut.  I don't see teacher pay at the same level as a Dr either but is 30k salary out of line for a college grad starting teacher? Remember that hundreds of people today got a surgeon who finished at the bottom of his class. He probably was paid the same as the person who finished at the top of his class.  A proctologist is not in it for the big paycheck either, but does it cause he loves it.  ;D

In Michigan...new teachers make an average starting salary of $34,724 and an average overall salary of $63,940.
If that is not good enough...try another state...or a different job altogether. But be happy...

Where I live if the teachers demand more money, our taxes are raised, and we are told the reason for it is education costs are going up. I haven't been able to figure out yet how this is saving taxpayers money.

When I worked Construction there was no such thing as bad construction workers. If you didn't do the job expected of you. You didn't have a job period. So please lets compare apples with apples. My brother is still in the heavy construction business, and its the same way. If you don't work your gone. Also if there are bad doctors they are run out of business by lawsuits. With teachers they practically have to commit murder, or molest a student, before a school district can fire them. When they are able to fire them, it costs the school district thousands of dollars to do so. 

My wife used to work for a nurse practitioner, and she said people on government access cards were in there all the time. She said some families were in three or four times a week. They would bring their kids in if they had a runny nose. The reason being they didn't have to pay for it. Just think on many are going to get free with Obama care. Shortage of doctors, long waiting times, reduction in quality care, Increased costs for those that pay. Its all coming under Obamacare.

Now we find out that the IRS computers don't interface with the Health care 'puters, so the 9% subsidy that IRS is suppose to kick in, is kicking out patients. And, a Harvard economics team, has looked at funding, and came up with a 700 billion shortage that the healthcare will have to ask for, before 2014. Yep, read it before ya pass it. gypsyman

Pattys heart DR just flat out quit being a dr, locked up the office, moved over 100 miles and said, I aint doing this. Talked to Pattys diabetes dr last week, his secretary said that when obummercare kicks in that he will be pd $45 per office call and be forced to accept it or quit, our family dr confirmed it. YEP, husseins plan is coming together, just as he planned. Thank a libbydem today. POWDERMAN.  >:( >:(

But when the bills start pouring in for the obama heath care the gov't will find they can't affort it and try to cut cost by regulating payments to the doctors. They will establish maximum payments doctors can get from the system  and the incentive to become a doctor will come crashing down........

In fact, if you want to stop the brain drain and boost the country's college grads, make college free to all students with a "C" average or above--- it would be cheaper to forgive outstanding student loans than what we gave Wall Street to bail them out.

The same thing happened in the pharmaceutical industry several years ago when the government saw how much medicare  and medicaide was costing and they changed the fee schedule (lowered it and slowed payment) that put a lot of pharmacies in the red and lots of them shut their doors or with the larger outfits they cut back lower preforming stores and cut back on the number of pharmacists or their amount of help.
