My dad was a machinist. That's some serious work there. Are those o-rings to center the sleeve in the bore?

Yes, and to keep it from scaring up the shot bbl. too...

Sometime back I read in a magazine (don't ask which one, I read so many I have no idea) anyway there was an article of a company that made full length inserts for a 12ga in 45-70 that were off center and could be turned as needed to regulate the two barrels to point at 75 yds and then locked in place. Of course as stated in one of the other posts loads would have to be on the mild side for most shotguns. If I run across it I will post it but don't get your hopes up.

I think there is more to regulating barrels than just turning them?

Not according to the article. The full length liners are made in an kind of out of round shape running the length of the liner, like thick on the right at the breach and thick on the left at the muzzle.
This way as you turn the liner the muzzle moves around in any direction so you should be able to move the point of impact anywhere you want, with in reason.
Makes sense to me anyway!

The problem I see with that is that if the barrel liner is drilled off- parallel to axis, as you turn the barrel to change it's windage, you also change it's elevation.

That part is true.
I don't recall if they make ones that are only off center on one end or not.
If they do then wouldn't that maybe answer the problem?

I had the same thoughts but, fortunately, landed on this Ugartechea SxS slug gun 4 or 5 years ago. It is regulated beautifully with the Rottweil Brenneke M.P. load (after trying many) that drives a 1 oz (440 gr) slug at 1500 fps. It will throw multiple pairs inside 2 1/2" at 50 meters right on top of the front sight and has done its job on deer and hogs. I may take it bear hunting next month here in Alberta. I have only seen one other but I would search hard and try to find one. It is a fun gun.

I have a Stevens (Double Barrel) 311-series H, I had it cut to 18.5 inch's and left the stock standard length.
I have played with a few rifled slugs in the gun at 25 yards. The Winchester 2 3/4 inch rifled slugs shot very well, about 2 inch group at 25 yards. I also shot some Remington slugs with similar results. I have not shot it any further than 25 yards. I did this because I wanted an up close gun. I though about using it on black bear on bait, but I have been having to much fun using my handguns to give it at try. I like the idea of the iron sights, mine just has the bead for a front sight.
