Simmons Pro Diamond Shotgun Scope

Three years ago I bought this scope, sighted it in, no problems.  Second year I shot all over the paper.  I had not dropped it or any rough treatment.  Returned scope to have it reworked.  I am using iron sights now, don't want the hassle of a broken scope again.  Anyone else had trouble with this scope

I use one (4x32) on a T/C Encore in 45-70. Never had a problem with it so far.
