Can't find the web form

HI,

I have an Encore I would like to list in FS but cannot find the short webform I need to complete to be in compliance with the GBO rules.  Can someone send me the link to this or tell me where to look...

Matt has a problem with "round tuits". His problem is he can never seem to find one so he doesn't get around to the things he sets out to do such as the electronic form to sign. I'm beginning to wonder at this late date if he is ever gonna get his "round tuit" on this one.  ???

Most likely all of his round tuits got burned up in his house fire, round tuits can be hard to come by usually, and fires don't help!! :o
