I have a "no-brand" scope that originally came with my Knight Vision BP rifle, on one of my Handi 45-70's.  It works really well.  I picked it up from Sportsman Guide.

It meets your criteria.  Call them and see if they sell it separately.

They also have this scope:

My recommendation if you are willing to spend a little more would be to pick up the Nikon "Omega" muzzleloader scope.  I have this on my Sidekick.

Muzzleloader trajectories are very similar to 45-70 trajectories.   Remember the 45-70 was originally a BP cartridge.  Shop around for a better price.

hughd - Sorry I missed the part where you stated: "mounts forward of the hammer".  None of the scopes mention mount forward of the hammer.  You'll probably have to get a longer eye relief scope.

The only variable power scopes I know of that will mount forward of the hammer are those short scopes with a 20mm objective diameter, Leupold and other make 1-4x20, Weaver used to make a 1-3x20 V3. There are several fixed 2½x and 4x scopes that work fine too, all of these require a long mount like the obsolete H&R Ultra rail, EGW or a modified Weaver #82, all shown in the FAQs.
