G 26 magazine failure....

Was cycling my G 26's defense ammo, new for old, today and during loading had one magazine push the bottom of the mag out leaving me with a handfull of base plate, pin plate, spring, follower, and ammo. Anyone else had this happen? Needless to say I'll not be using that mag anymore!

Standard Glock base plate or a Pearce Grip Extension? Had trouble with Pearce +2 ext. on my G19 years ago.
