pierced primer?

I had a 454 casull barell rechambered to the 460 s&w by ssk. The 460 is also supposed to be safe with the 454 and 45 colt loads.  I also (not by ssk) had a trigger job with heavier hammer spring installed.  I shot 5 shots of 454 loads (factory) today and had 1 with a pierced primer.  Could this  be a result of the heavier hammer spring? Just dumb luck with the ammo?  spacing problem?   Any ideas?

are you shooting factory or handloads?

these are factory 454 casull, hornady 240 xtp's
